A patient walks in. They want to see you. They're ready to pay. And you send them somewhere else.

That sounds like a business mistake. Michael Lau, Co-Founder and CEO of [P]rehab (@theprehabguys) and [P]rehab Physical Therapy, would tell you it's the business model.

We're in a strange moment for independent operators. The clinical knowledge your clinicians spent years accumulating — the differential diagnoses, the exercise progressions, the patient education — is now available to anyone with a phone and thirty seconds. AI will generate a rehab program on demand. YouTube will demonstrate the technique.

So if information is the product, you're selling a commodity. And commodities compete on price until the margin is gone.

The practices already thriving have figured out the defensible product was never information. It was trust. And trust requires a different operating model: different staffing priorities, different scheduling logic, different success metrics, and a willingness to turn away revenue that undermines the relationship.

A REAL PRACTICE STORY

Michael Lau's doesn’t see patients if they don’t fit the criteria he has defined. His reasoning is it builds trust. It registers to patients when you are serving their best interests and are invested in the best possible outcome for them. Running this as a system rather than a sentiment takes six things.

Define who you keep

Conditions, goals, acuity levels, red flags — written down, specific enough to actually screen someone out.

Move the screen before the first visit

Once a patient is in the room, the honest conversation costs everyone more. The screen belongs on the phone or in the intake review, and it answers one question: Can we genuinely help this person?

Build the destination before you need it

"I'm not the right fit" builds nothing. A specific name, with a note already sent, builds everything. Every clinic director needs a network of providers for their own market.

Give your staff permission to say no

If intake is measured on booking rate, the protocol dies in week one. Give them explicit authority, the actual language to use, and credit for referrals out in the same meeting where you review conversions.

Stay the coach

You're not disappearing, you're quarterbacking. Lau's instruction to his own therapists: "You are this person's coach, you are their health coach.”

Identify the right metrics

Throughput measures the thing being commoditized. Patient-initiated returns, word-of-mouth share, completed plans of care. None of it moves in a month. All of it compounds.
